Many years ago was I right to find a way to avoid a character using all his Karma for causing the death of A Npc when the Npc that died only died because I as The GM forgot to give the character information that would have guaranteed that The Npc didn't die?

First, the cops are not required to be language police and grammar scholars. It's not on them to read the warrant.
Second, the idea of technicalities getting someone off the hook is a TV trope that I encourage you to ignore.
Third, I'd try to remember this is a game. Let the characters sort this out. If someone has the Law talent, let them make a roll against some sort of intensity

But that doesn't answer the question of what if the hero thinks he or she's responsible but isn't, after all its not accidental death or mysterious death, he or she believes he or she was in some way responsible when in fact he or she wasn't in any way responsible for the death

We all know that if A Hero cause's someone's death or is responsible for a death or someone dies because of something they either did or didn't do that they lose all their Karma, as do all their allies
But then I had a thought, what if the hero isn't responsible for someone dying but he or believes that they are responsible for that person dying?, after all even Super Heroes make mistakes
